Virginia Retirement System Durable Power of Attorney (VRS-901)

  • If you need a person to act on your behalf, complete and have notarized the VRS Durable Power of Attorney (VRS-901).   
  • This option is available for VRS members and Norfolk Employees' Retirement System members with basic life insurance administered by VRS. It ensures that your designee can manage your VRS benefits without interruption and according to your wishes.
  • Please send the completed form directly to VRS for processing. Do not return it to the City.
